Street Connections is the flagship service of the Winnipeg Regional Health Authority's (WRHA) Healthy Sexuality and Harm Reduction (HSHR) team. It is a harm reduction and nursing van that drives around the city of Winnipeg six nights a week.

Safer Crack Use Kit Distribution in the Winnipeg Health Region

Safer Crack Use Kit provider survey

Survey tool administered to health care and outreach providers in Winnipeg who distribute safer crack use kits to clietns. Results can be found in the Street Connections evaluation report (accessible through Sage). The tool is adapted from the one used by Vancouver Coastal Health for their 'Evaluation Report: Vancouver Coastal Health Safer Smoking Pilot Project.'

Safer Crack Use Kit client survey

Survey tool administered to Street Connections clients who use crack. Results can be found in the Street Connections evaluation report (accessible through Sage). The tool is adapted from the one used by Johnson, Malchy, et al.'s 'Lessons Learned from the SCORE Project' report.

Street Connections Evaluation Report

Evaluation report of the Street Connections mobile harm reduction service. Includes two main sections:

1) Program monitoring report for the year October 1, 2013 to September 30, 2014

2) Evaluation of the distribution of Safer Crack Use Kits (SCUKs)

Outreach/Street Connections Logic Model

Logic model developed by outreach staff and community partners. The model includes all activities run by our staff via the Street Connections mobile harm reduction service.
